School Health

There are NC immunization requirements for students entering Kindergarten, 7th grade, 12th grade, or an NC public school for the first time.

● For 7th graders, the requirement is for Tdap and Meningococcal.
● For 12th graders, the requirement is for the Meningococcal booster.

NC law also requires submission of a current health assessment for Kindergarten students and other students entering an NC public school for the first time.

We must have an official immunization record and health assessment from your child’s healthcare provider by the 30th calendar day for your child to remain in school.

Please schedule appointments as soon as possible and contact your child’s medical provider or school nurse if there are questions.

As a reminder, if your child had medication at school last year, please remember you will need to have your physician complete a NEW medication form to continue school administration for the 2026-27 school year. Contact your school for a copy of the form. Medical action plans will be sent home at the beginning of the year for you to review, sign, and return to school.

If your child had a diet order last year, to continue diet modification for the 2026-27 school year, this will need to be updated as well. This form can be found on the Stanly County Schools website under Child Nutrition.

Please contact your child’s school if you have any questions or concerns.
